The Truth About Liposuction: What It Really Does—and What Most Patients Get Wrong

June 24, 2026 by Allison Gross

Liposuction has been one of the most requested cosmetic procedures for decades. Despite its popularity, many misconceptions still surround what liposuction actually accomplishes and how modern technology has changed the procedure.

In a recent episode of The Plastic Surgery Revolution, board-certified plastic surgeon Dr. Steven Davis explained why liposuction remains one of the most effective body contouring treatments available—and why it’s more advanced than ever.

Liposuction Permanently Removes Fat Cells

At its core, liposuction is straightforward.

The procedure removes fat cells from specific areas of the body. Once those fat cells are removed, they are gone permanently.

This is an important distinction because many people believe that fat simply “moves” somewhere else after liposuction. In reality, only the treated areas experience a reduction in fat cells.

If a patient gains weight after liposuction, the remaining fat cells throughout the body can still enlarge. However, the treated area will always contain fewer fat cells than it did before the procedure.

The Myth of Fat Coming Back

One of the most common questions Dr. Davis hears is:

“If I get liposuction on my abdomen, will the fat come back somewhere else?”

The answer is more nuanced.

The abdomen will always have fewer fat cells after treatment. However, if weight is gained later, untreated areas—such as the arms, thighs, or back—may become larger because those fat cells remain intact.

Understanding this concept helps patients set realistic expectations and appreciate the long-term benefits of targeted fat removal.

Why Planning Matters

According to Dr. Davis, successful liposuction isn’t just about removing as much fat as possible.

It’s about understanding how the body will continue to age.

Fat plays an important role in facial fullness, body contours, and overall balance. Removing too much fat in certain areas may create concerns years later as natural aging, weight fluctuations, or GLP-1 medications contribute to volume loss.

That’s why consultation and treatment planning are critical.

The goal isn’t simply to remove fat today—it’s to create a result that still looks natural years from now.

Modern Liposuction Is About Sculpting

Today’s liposuction procedures are dramatically different from earlier generations.

Advanced technologies now allow surgeons to combine fat removal with body sculpting and skin tightening.

Technologies such as:

  • Radiofrequency-assisted liposuction
  • Ultrasound-assisted liposuction
  • Energy-based body contouring devices

allow surgeons to selectively remove fat while improving skin contraction and contour.

Rather than simply reducing volume, modern liposuction helps create shape, definition, and balance.

Liposuction and GLP-1 Weight Loss Medications

As medications like Ozempic®, Wegovy®, and tirzepatide continue to help patients achieve significant weight loss, many people are discovering that certain areas remain resistant to change.

Even when the scale reflects success, stubborn fat deposits may remain in areas such as:

  • The lower abdomen
  • Inner thighs
  • Arms
  • Love handles

In these cases, liposuction can provide targeted improvement without requiring additional weight loss.

Dr. Davis emphasizes that patients don’t necessarily need to keep losing weight if they’re happy with their overall progress. Instead, liposuction can address the specific areas that diet, exercise, and medications often leave behind.

The Future of Body Contouring

While the basic principle of liposuction remains unchanged—permanent fat cell removal—the tools available today continue to evolve.

The combination of fat reduction, contour enhancement, and skin tightening allows surgeons to deliver more refined, natural-looking results than ever before.

For patients considering body contouring, understanding both the immediate and long-term effects of liposuction is essential.

Because the best results aren’t just about looking better today—they’re about aging beautifully for years to come.
