A revised Notice of Privacy Practices may be obtained by forwarding a written request to Steven L. Davis, D.O., F.A.C.O.S. Privacy Officer at 1916 Rt. 70 E., Suite One, Cherry Hill, NJ 08003.

With this consent, Steven L. Davis, D.O., F.A.C.O.S., may call my home or other alternative location and leave a message on voicemail or in person in reference to any items that assist that practice in carrying out TPO, such as appointment reminders, insurance items and any calls pertaining to my clinical care, including laboratory results among others.

With this consent, Steven L. Davis, D.O., F.A.C.O.S. may email to my home or other alternative locations any items that assist the practice in carrying out TPO, such as appointment reminder cards and patient statements. I have the right to request that Steven L. Davis, D.O., F.A.C.O.S. restricts how it uses or discloses my PHI to carry out TPO. However, the practice is not required to agree to my requested restrictions, but if it does, it is bound by this agreement.

By signing this form, I am consenting to Steven L. Davis, D.O., F.A.C.O.S. use and disclosure ofmy PHI to carry out TPO. I also acknowledge that I have received a paper copy of Steven L. Davis’s Notice of Privacy Practices. I may revoke my consent in writing except to the extent that the practice has already made disclosures in reliance upon my prior consent.

I hereby give permission to Davis Cosmetic Plastic Surgery to use the video I uploaded to specified social platforms for the contest. I authorize him to use such photographs and video documentation for public interest, marketing material, and media promotions.
In addition, I give permission to Dr. Steven Davis and/or his staff to use my photographs/videos for prospective patient viewing. My photographs/videos will be used to educate future patients on particular procedures/surgeries and for them to have a better understanding of the outcome.
